Northern Health are seeking applications from motivated and suitably experienced Allied Health Assistants to join our Community Therapy Services (CTS) team.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.

Services are provided through our five main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Community Hospital, and Kilmore District Hospital.  In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.

About the Role:

An exciting part-time opportunity exists for an Allied Health Assistant (AHA) to join our team at Northern Health.

This role requires an AHA who is passionate about providing high quality person-centred care. Working with both Occupational and Physiotherapists within the Community Therapy Services (CTS) at Craigieburn Community Hospital, you will deliver individual and group-based therapy through the application of competent clinical treatment skills.

This is a fixed term role (until February 2026), working 45.6 hours per fortnight.

To be successful in this role you will have:

• Certificate IV AHA Qualifications (or equivalent)
• Current Victorian Driver’s License
• Current National Police and Working with Children History Check
• Proven ability to work collaboratively within the multi-disciplinary team
• Commitment to lifelong learning
• Demonstrated initiative with strong effective communication skills

Dialysis Experience Required

Must have a Current CCHT

The PCT will work on-site at our dialysis den at St. Pauls in Belleville, IL.

Please note that the PCT may need to float to other facilities.

 Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have significant experience within DaVita, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to GSD. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.

What you can expect:

In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care.

Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.

Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners.

Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your "core" skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. There's a lot to track and manage and the joy comes in witnessing how your care impacts patients' lives.

Patient Care Management

Receive patients with warmth and provide a comfortable atmosphere for assessment and treatment.

Provide info related to services and tariff to the patients.

Orients and educates patients and their families by explaining the care plans and other information relating to the treatments.

Pre and post-counseling for the patients undergoing the treatment plans if required.

Address and resolve patient complaints and concerns promptly and effectively.

Understand and action accurate data entry on a daily basis

Handling Appointments

Understand the CRM platform

Comprehend detailed nuances around therapist availability and accordingly block appointments

Maintain data related to financial transactions, patient flow, and individual patient visits.

Track patient's journey from lead status to discharge and follow-ups

Patient Experience Enhancement

Respect patients by recognizing their rights; and maintaining confidentiality.

Collect patient testimonials and feedback as per organizational policy.

Monitor and evaluate patient care metrics, identifying areas for improvement.

TAT of 30 mins to be adhered for all the calls received

Coordination and Collaboration

Responsible to follow up on the assessment report till it gets delivered to the patient

Work closely with the sales team to promote patient care initiatives and success rate

Increase in conversion ratio will be reviewed in constant interval

Quality Improvement

Monitor delivery of care by documenting; identifying progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; and reviewing the care plan.
