We are seeking an experienced and detail-oriented Surgery & Pathology Medical Coder responsible for accurately reviewing medical records and assigning appropriate CPT, HCPCS, and ICD-10-CM codes for surgical procedures and pathology services. The role requires a strong understanding of surgical documentation, pathology reports, coding guidelines, modifiers, bundling rules, and payer-specific requirements.

Key Responsibilities: (Surgery with Pathology Experienced Coders)

1. Review and analyze operative reports, surgical notes, pathology reports, physician documentation, and other relevant medical records.
2. Assign accurate CPT, HCPCS, and ICD-10-CM codes based on documented services and diagnoses.
3. Perform coding for various surgical procedures, including outpatient and inpatient surgical cases, as applicable.
4. Review pathology and laboratory-related documentation and accurately code pathology services based on the procedures performed and documentation available.
5. Apply appropriate CPT modifiers , including but not limited to 25, 26, 50, 51, 52, 53, 59, 76, 77, and 78, wherever applicable.
6. Identify and apply appropriate global surgery package guidelines and postoperative services coding rules.
7. Ensure accurate coding of surgical procedures, biopsies, excisions, resections, endoscopic procedures, and other applicable specialties.

Stay updated on annual changes to CPT, HCPCS, ICD-10-CM, AMA guidelines, CMS regulations, NCCI edits, and payer-specific policies.

Work closely with auditors, QA teams, billing teams, and operations teams to resolve coding-related issues.

Maintain strict confidentiality of patient information and comply with HIPAA and applicable data privacy regulations.

Candidates with experience in US healthcare medical coding and RCM processes will be preferred.
