• Conduct comprehensive assessments of seniors' needs, including physical, social, and emotional well-being, to develop tailored care plans.
• Coordinate and schedule a range of support services, such as in-home care, medical appointments, transportation, and social activities.
• Liaise with healthcare providers, social workers, and other community resources to ensure integrated care for clients.
• Provide emotional support and advocacy for seniors and their families, acting as a primary point of contact.
• Monitor the effectiveness of care plans and make adjustments as necessary based on client progress and evolving needs.
• Educate families and caregivers on available resources and best practices for elderly care.
• Manage client records and documentation, ensuring accuracy, confidentiality, and compliance with regulations.
• Organize and facilitate support groups or social events for seniors to foster community engagement.
• Handle crisis interventions and emergency situations with professionalism and promptness.
• Collaborate with the broader care team, participating in case conferences and contributing to service improvements.
• Maintain strong relationships with local service providers and community organizations.

• Bachelor's degree in Social Work, Gerontology, Nursing, Psychology, or a related field.
• Minimum of 5 years of experience in social services, healthcare, or community care, with a focus on elder care.
• Deep understanding of the challenges faced by seniors and their families.
• Knowledge of local health and social care systems, resources, and support services.
• Excellent assessment, planning, and problem-solving skills.
• Strong interpersonal and communication abilities, with empathy and cultural sensitivity.
• Proficiency in using care management software and standard office applications.
• Ability to work independently and manage a caseload effectively in a hybrid work environment.
• Willingness to travel locally for client assessments and home visits.
