Our client, a leading provider of community-based healthcare services, is seeking an experienced and compassionate Clinical Nurse Manager to oversee their operations. This vital role involves leading a team of dedicated healthcare professionals, ensuring the highest standards of patient care, and managing the day-to-day operations of our community care services. You will be responsible for staff supervision, patient assessment, care planning, and ensuring compliance with all regulatory and clinical guidelines. The ideal candidate will possess strong clinical skills, exceptional leadership abilities, and a deep commitment to serving the community.

- Lead, mentor, and manage a team of nurses, healthcare assistants, and support staff.
- Oversee the provision of high-quality patient care in community settings, including home visits and clinic-based services.
- Conduct comprehensive patient assessments, develop individualized care plans, and ensure their effective implementation.
- Monitor patient progress and outcomes, making adjustments to care plans as necessary.
- Ensure adherence to all clinical protocols, safety procedures, and regulatory requirements.
- Manage staff scheduling, performance evaluations, and professional development.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Maintain accurate and timely patient records and documentation.
- Manage resources efficiently, including supplies and equipment.
- Act as a primary point of contact for patients, families, and external healthcare providers.
- Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN) or equivalent qualification.
- Master's degree in Nursing, Healthcare Administration, or a related field is preferred.
- 5+ years of clinical nursing experience, with at least 2 years in a supervisory or management role.
- Experience in community health, home healthcare, or public health nursing is highly desirable.
- Strong leadership, decision-making, and problem-solving skills.
- Excellent communication, interpersonal, and patient advocacy skills.
- Proficiency in electronic health record (EHR) systems.
- Knowledge of relevant healthcare regulations and standards.
- Current nursing license and any required certifications.
- Ability to work effectively in a dynamic and challenging environment.
