- Provide training, mentorship, and ongoing support to CHWs.
- Develop and implement program strategies to reach target populations.
- Oversee the delivery of health education, screening, and referral services.
- Ensure CHWs adhere to program protocols and ethical guidelines.
- Monitor program activities and collect data to assess effectiveness and impact.
- Collaborate with healthcare providers, clinics, and community organizations.
- Develop reports on program progress and health outcomes for stakeholders.
- Identify barriers to healthcare access and advocate for community needs.
- Contribute to the development of culturally appropriate health resources and materials.
- Bachelor's degree in Public Health, Nursing, Social Work, or a related field.
- Minimum of 5 years of experience working as a Community Health Worker or in a related public health role.
- At least 2 years of experience in a supervisory or leadership capacity.
- Strong understanding of community health principles and practices.
- Experience with health education, outreach, and case management.
- Excellent leadership, communication, and interpersonal skills.
- Proficiency in data collection and reporting tools.
- Ability to work independently and manage time effectively in a remote setting.
- Culturally competent and sensitive to the needs of diverse populations.
