About the role

Reporting directly to the Team Leader – Clinical Nurse Consultant, you will be responsible for:
- Undertaking a high standard of nursing assessment utilising standardised nursing assessment tools appropriate to the clinical presentation
- Ensuring triage of referrals and timely allocation of program resources for appropriate referrals
- Providing detailed care planning following assessment, using a person centred and multidisciplinary team approach
- Providing information including psycho-educational and psycho-social interventions and resources about complex health symptoms associated with dementia, end stage dementia and end of life to people diagnosed with dementia, their families and carers.

About you
To be successful in this role, we are looking for someone with the following:
- Tertiary qualification in Nursing is required; Post graduate studies in Palliative Care and / or Dementia Care will be highly regarded
- Registered Nurse with 5+ years postgraduate experience with current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Understanding of dementia and ability to provide comprehensive nursing assessment and consultation for people with dementia, their families and carers
- Demonstrated knowledge and experience of clinical presentations and complex comorbidities of people with dementia in late and palliative stages of dementia

About the Role:
The Clinical Support Nurse is an important member of the Education team, a role model of excellence in clinical practice and encouraging life-long learning. Supporting all level of learners and staff.
A member of the Kilmore District Health (KDH) site Education team, this role will assist in anticipating and responding to the clinical developmental needs of the students and staff across the organisation. Clinically based, the position will liaise closely with the Clinical Education Coordinator, Nurse Unit Managers and other members of the Education team to assist and support staff and students in safe practice. This role is a clinical leadership position that involves aspects of program delivery, performance review through formative and summative feedback, and completion of appropriate appraisal processes as deemed by the relevant NH requirements. This includes performing assessments for mandatory and clinical skill development as required across the health service.
This position is fixed term part time (32 hours per fortnight) until February 2026
This position is responsible for: providing “bedside” support. This may include but is not limited to:
- Demonstrating a high level of clinical decision making with an emphasis on problem identification and solution as well as analysis and interpretation of data
- Maintaining and improving clinical standards while being a positive role model across the organisation
- Identifying immediate and ongoing learning and development needs of staff and students in consultation with NUMs, Clinical Nurse Educators and other members of the team
- Promoting evidence-based clinical practice, within existing policies and procedures, while supporting staff across the organisation
- Assisting and encouraging staff to achieve their mandatory training requirements.
- Encouraging and supporting clinical staff in their ability to provide appropriate support and guidance for entry level students, graduates and post-graduate nurses
- As directed and supported by the Clinical Education Coordinator, contributing to education/training programs and relevant assessments (such as formative and summative evaluation of program participants)
