About the Role:
Patient Support Services provides support for the patients of Canberra Hospital through a range of services which includes Wards persons, Hospital Assistants, Central Equipment & Courier Service, Ward Clerks and Patient Liaison and Admission Team.
The Central Equipment & Courier Service (CECS) is a small team responsible for the distribution, maintenance, and record management of various forms of clinical equipment, as well as couriering Pathology specimens and blood products across the Canberra Hospital & Health Services campus using a clinical work device (DHR Rover Device) and Territory Radio Network (TRN) system.
Under the direction of the CECS Team Leaders and/or Operations Manager the Equipment & Courier Officer will work closely with a dedicated and skilled team to deliver a safe, efficient, and effective service for inpatients and associated clients at The Canberra Hospital.

We at LONGENY are seeking a compassionate and detail-oriented Patient Coordinator and customer support individual to join our team. As a Patient Coordinator, you will play a vital role in assisting patients with their medical needs throughout their journey in our healthcare facility.
Your responsibilities will involve developing personalized care plans, coordinating various patient care services, and collaborating closely with patients and ensure that they receive the necessary wellness treatment and support.
Working alongside a team of dedicated medical and other professionals, you will strive to deliver the highest quality of care and ensure a positive patient experience.
Your commitment and expertise will contribute to the overall well-being and satisfaction of our patients. You will actively help patients learn better about their current health status and guide them through various health situations. Your duties will also include listening to clients, asking necessary questions, and developing effective strategies to improve their lives.
Excellent verbal and written communication skills are essential, along with the ability to coordinate, be friendly, and professional with outstanding people skills
**Responsibilities**:
- Primary responsibilities include but are not limited to, patient phone screenings, and scheduling appointments. The Patient Cordinator will be responsible for educating the patient on procedures through Zoom or in-office consultations
- Patient enrolment and support patients with the documentation process (wherever applicable)
- Scheduling home visits to patient's home as per the approved process flow wherever applicable
- Documents all patient health information, such as medical history, examination findings, and investigation results, in the electronic medical record, in a timely and accurate manner
- Coordinate and implement specialist referral recommendations as appropriate and necessary
- Participate in health education for all patients, through individual patient counselling and group health talks
- Counsel the patients on call and physically for various treatments, costs, diagnostics, therapies etc.
- Follow up with the already connected patients on a regular basis so as to convince for further engagement.
- Talk to the Doctor/Coordinator, and gather information about treatment offered, and collate information to be used for patient communication.
- Report the status of all the patients connected, actions taken, to the management on a daily basis for all teams.
- Work with the physician to design a partnership for long-term health care with every patient
- Participate in all Clinic Practice Meetings, external clinic activities, and meetings/ committees as needed **Requirements**:
**UG**:M.Sc / B.Sc in Nursing
A minimum of 3 years of proven work experience as a customer support or patient cordinator or a similar role
Superb verbal and written communication skills.
Aptitude to empathize with clients and help them open up.
Excellent customer service skills and the ability to handle multiple tasks
Good time management skills.
Proven experience in developing effective treatment plans
Pay: ₹22,500.00 - ₹25,000.00 per month
