Clinical Nurse Specialist

We have an exciting permanent part-time, 19 hours per week, opportunity for a Clinical Nurse Specialist (CNS) who will deliver high-quality clinical services directly to customers, both digitally and face-to-face. The CNS will provide expert advice to the organisation’s staff and external health professionals regarding customer needs and support the development of staff to enhance service delivery. Additionally, the CNS will take a leadership role in clinical education, foster evidence-based practices, and drive quality initiatives to ensure outcomes-focused services. The CNS will coordinate clinical nursing activities and monitor customer outcomes, ensuring their care aligns with individual needs and goals.

The CNS will:

  • Collaborate directly with customers to assess, plan, and deliver personalised nursing services that align with their individual goals.
  • Review and monitor clinical care to ensure it aligns with clinical plans, providing guidance and direction to teams to prioritise the safety, health, and wellbeing of customers.
  • Deliver clinical nursing services in accordance with external legislation, practice standards, and the organisation’s policies and procedures
  • Provide advice and support to staff regarding client needs, working together to identify solutions that promote client choice, uphold their human rights, and meet their specific requirements.
  • Conduct clinical assessments and contribute to the development of care plans that support the long-term wellbeing of clients with complex needs, collaborating with the client and team to evaluate and adjust these plans to ensure they continue to meet client goals.
  • Support staff in delivering effective, client-centred clinical care that adheres to best practice standards, providing both formal and informal training and coaching as necessary.

About you

To be successful in this role, you will have:

  • Current Australian Health Professional Registration Authority registration for a Registered Nurse (RN) Division 1.
  • Minimum five years nursing experience with three years in the disability or community services sector.
  • Proven ability to collaborate with various roles, including non-registered health professionals, to enhance their understanding and skills, ultimately improving customer outcomes.
  • Strong time management skills with the ability to prioritise and manage competing demands effectively.
  • Experience working in an environment that prioritises consumer needs and expectations, while upholding and respecting human rights.

    Please note that the PCT may need to float to other facilities.

     Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have significant experience within DaVita, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to GSD. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.

    What you can expect:

    In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care.

    Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.

    Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners.

    Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your "core" skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.     DaVita is seeking a Patient Care Technician who is looking to give life in an outpatient dialysis center.   • Joining DaVita as Patient Care Technician (PCT) is an exciting opportunity to jump start your career in the health care industry! With paid, extensive training, you will gain the skills needed to care for our patients and build a network of other health care professionals across the geography where you live. Once you have successfully completed training, you will join a holistic team of care professionals – including nurses, dietitian, social workers, and other experienced PCTs – to care for our patients.     Some details about this position:

  • No Dialysis experience is required.

  • Training may take place in a facility or a training clinic other than your assigned home clinic

  • Potential to float to various clinics during and after your training

  • You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays

  • What you can expect:

  • Direct Patient Care. Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.

  • Our PCTs care for multiple patients.

  • Technician Duties. To monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. Please note you will have exposure to blood and needles.

  • Building long-term relationships with your patients who are suffering from end stage renal disease and are receiving treatment on average 3 times/week. You will also be responsible for educating patients on Kidney Dialysis related topics.

  • Team. Cross functional team of clinicians including technicians, nurses, dietitians, social workers and leadership. PCTs work under the supervision of a RN.

  • Desire to enter the health care field to care for other people in need

  • High school diploma or equivalent

  • Must be comfortable working around blood and needles

  • Must be comfortable mixing acid or bicarb

  • Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet so wear comfortable shoes! Our work ends when it's safe for our patients.

  • Willingness to train and work across multiple clinics within the territory as needed.

  • Health care experience a plus including: BONENT Certified, Certified Hemodialysis Technician (CHT), Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or Phlebotomy certification.
