About the role:

This role is focused on delivering high-quality, safe, and person-centred 1:1 support to people living with a disability. Support Workers play a key role in helping clients achieve their personal goals by promoting independence, choice, and control. In our Community Programs, this includes transporting clients using your own vehicle to support their participation in everyday activities and community engagement.

 You will be responsible for:

  • Providing tailored support based on individual My Support Plans
  • Assisting with goal-setting and implementation of support strategies
  • Delivering care aligned with Behaviour Support Plans and NDIS Practice Standards
  • Supporting clients with high-intensity needs, including subcutaneous injections (insulin) where authorised
  • Completing accurate, timely documentation and case notes
  • Champion choice, dignity, and cultural respect in all client interactions
  • Ensuring compliance with Afford policies, safety standards, and legislative requirements
  • Reporting incidents and manage risks responsibly
  • Upholding Afford’s values and contributing to a culture of inclusion and empowerment

About you:

In addition to your passion for supporting clients in the community, we see you as having:

  • Certificate III or above in Disability, Aged are or Community Services (or currently studying)
  • Valid First Aid Certificate
  • Competence in high-intensity supports (training provided by Afford)
  • Current Australian Drivers Licence
  • Use of own vehicle to provide client transport for community – based duties
  • Strong communication and interpersonal skills
  • Confidence using Microsoft Office and Client Information Systems
  • All mandatory checks and screenings as per Worker Screening Policy
  • Ability to prioritise tasks, think critically and work flexibly
  • Ability to complete Pre-Employment Medical/Functional Assessment (paid by Afford)
  • COVID vaccination certificate or a medical exemption

The service is required to promote individual and community wellbeing and reduce substance abuse through the provision of holistic, culturally appropriate alcohol and other drug harm minimisation, education, treatment, rehabilitation and after care services.

The Community Support provides for the integration of cultural authority and culturally safe practices in assisting the program team deliver services to the community, by assisting to establish and maintain positive relationships with individuals, families and communities. The role is required to maintain a link between Wurli and their clients who are utilising our services and act as role model for Aboriginal and Torres Strait Islander people by modelling appropriate healthy lifestyle behaviours within a harm minimisation and reduction framework.

Ideally we are seeking candidates with the following
