Qualifications:
  • Bachelor's or Master's degree in Nursing, Social Work, Gerontology, Public Health, or a related field.
  • Minimum of 5 years of experience in a healthcare or social services setting, with at least 2 years in a supervisory or management role.
  • Proven experience in developing and implementing care plans for the elderly.
  • Strong understanding of geriatric care needs, chronic conditions, and dementia care.
  • Excellent leadership, team management, and interpersonal skills.
  • Exceptional communication, problem-solving, and decision-making abilities.
  • Proficiency in electronic health records (EHR) systems and case management software.
  • Ability to work independently and collaboratively in a hybrid work environment.
  • Valid nursing license (e.g., RN) or relevant professional certification is highly preferred.
  • Compassionate and empathetic approach to caregiving.
